Please ensure that all the documents (FIVE  items) below are attached with your application form before submitting it to the Secretariat of Universiti Kebangsaan Malaysia Animal Ethics committee (ÌÇÐÄÔ­´´AEC), application submission is via EMAIL : ukmaec@ukm.edu.my

1. ÌÇÐÄÔ­´´AEC application form.
2. Full publication if any (Minimum 1 publication – Maximum 3 publications).
3. Provide flow chart on summary of study design.
4. Provide your contact number and email address.
5. Payment Proof.

*For the application from PPÌÇÐÄÔ­´´, there is additional Infos required. This is for the Medical Research Secretariat of the Faculty screening procedure.

The additional Infos are :

1. Document of Borang Permohonan Menjalankan PenyelidikanÌý(±«°­²Ñ-³§±Ê±Ê±õ-µþ°¿01)
2. Perjanjian Kerahsiaan (Non-Disclosure Agreement) – ÌÇÐÄÔ­´´ Student ONLY
3. Document of Research Proposal
4. Publication Policy (completed and signed by all researchers involved) 
5. Research Agreement (Draft – if relevant)
6. Any form of confirmation from the sponsor (outside grant)
7. Checklist for Proposal Acceptance by the office
8. A letter from an outside organisation (other than ÌÇÐÄÔ­´´) for research permission (if relevant )
